Description: 2nd jump of the weekend, from 14k out of a King Air. Jumper was doing a 2-way, and was smiling and waving at breakoff (4200). Same type of deployment system had been used for the last 11 jumps, though different container systems had been used. Her altimeter was reading correctly at breakoff, and she had a TimeOut audible altimeter in her factor diver. She was found with no handles pulled, with the reserve pilot chute slightly extracted and the main bag just out of the container with all lines stowed. The pilot chute moved easily in the the pocket, and her long manicured fingernails showed no chipping or scratching to indicate any struggle with handles. She was observed by two non jumpers to be in a stable, boxman position turning slowly just prior to impact.
Lessons:Unfortunately, this goes in the category of a simply unexplicable no-pull. Friends indicate there is no reason to suspect suicide in this incident. An AAD would have almost certainly prevent this accident.
